Rx Rewards Visa® Prepaid Card: Incentive Reward FAQs MAKING A BEHAVIOR CHANGE Q: Does my new prescription request need to be approved to receive the incentive reward? A: Yes. If your prescriber does not approve the new prescription, you will not be eligible for the reward incentive. We will communicate to you if your request is denied and the reason why. If it is because you need to be seen first, and after seeing your prescriber they approve the new prescription, please call us and let us know so we can renew your eligibility for the incentive reward. Q: Can I receive more money for making additional medication switches? A: Yes. If you have multiple medications that are eligible for an incentive, you can receive multiple awards. If you make additional switches after you receive your reward card, you will receive a new Visa Prepaid Card. GENERAL PROGRAM Q: Will a new card be issued for each reward? A: Yes. If a member is eligible and approved for an additional Rx Reward the member will receive a new card. Q: Is there any cost to use the card? A: No, this is a free program for qualified members. Q: What happens if I lose my card? A: If a card is misplaced or stolen the member should call Customer Service at 1-877-227-0956 immediately and provide the Card number. If you call before the expiration date, you will be sent a new Card with the remaining balance at the time of your call. Fees may apply. Refer to Cardholder Agreement for details. Q: When will I get my card? A: Your Rx Rewards Visa Prepaid Card will be mailed to you via USPS, after you have picked up or received your new prescription. U.S. mail can take up to 2- 4 weeks to arrive at your home. Q: Where can I use my Visa Prepaid Card? A: You can use your Visa Prepaid Card any place Visa debit cards are accepted (except gas pumps, ATMs, cash access and for recurring payments) with the following usage tips:
Q: How do I activate my account? A: Members can activate and manage their card at www.MyPrepaidCenter.com. Q: Do the reward dollars expire? A: Yes.
